Endometriosis is a chronic, inflammatory condition characterized by the implantation of tissue that makes up the inner lining of the uterus (the endometrium) in organs outside the uterus, such as the ovaries, fallopian tubes, and the peritoneum. When it implants in the ovaries, it is commonly referred to as a “chocolate cyst” (endometrioma).

How Does Endometriosis Affect Fertility?

Endometriosis tissue continues to bleed during menstrual periods, just like the inside of the uterus. This blood that accumulates in the abdomen can cause adhesions, blockage of the fallopian tubes, and a decrease in ovarian reserve. Additionally, the inflammatory environment it creates can negatively affect egg quality and the embryo’s ability to implant in the uterine wall.
“The most important rule in managing endometriosis is to preserve ovarian reserve. Since unnecessary and aggressive cyst surgeries can reduce this reserve, the priority should be egg or embryo freezing.”

Most Common Symptoms

  • Severe Menstrual Cramps (Dysmenorrhea): Cramps severe enough to disrupt daily life.
  • Chronic Groin and Lower Back Pain: Pain that persists even outside the menstrual period.
  • Painful Intercourse (Dyspareunia): A sensation of deep pelvic pain during intercourse.
  • Difficulty Conceiving: Failure to conceive within 6–12 months of unprotected intercourse.

Umay IVF Treatment Strategy

  1. Precise Assessment of Ovarian Reserve: Ovarian capacity is mapped through an AMH test and an antral follicle count via ultrasound.
  2. Personalized Medication Protocols: Specialized protocols are implemented to suppress inflammation and support the development of high-quality eggs.
  3. Advanced Laboratory Support: Following egg retrieval, embryo monitoring is performed using ICSI and time-lapse (Embryoscope) incubators.
  4. Minimally Invasive Laparoscopy When Necessary: Tissue-preserving laparoscopic surgery is preferred only in cases where the fallopian tubes are blocked or severe pain is present.
